Home Care Packages
Your Home Care Package is coordinated according to your individual needs, designed to support people who have varying levels of care needs.
All packages are delivered using a Consumer Directed Care (CDC) approach, which puts you in control of the services you receive, allowing you to combine the services you require into one package.
Your Home Care Package is tailored to meet your individual needs and may include:
• Personal care
• Meal preparation
• Transport to appointments
• Social support
• Cleaning
• Consumables, such as continence and catheter supplies and wound care products
• Shopping
• Gardening
• Equipment hire and purchase
• Nursing
• Massage therapy
• Allied health services, such as physiotherapy, hydrotherapy and podiatry
Your coordinator can also make referrals on your behalf for higher level package assessments with the Aged Care Assessment Team (ACAT), community centre referrals and the Continence Aids Payment Scheme.
